Credit Card on File Policy

We are starting a new policy requiring a credit card on file. As you may be aware, the current healthcare market has resulted in insurance policies with higher out of pocket costs to you, the insured. Some plans require deductibles and copayments in amounts unknown at the time of your visit. Similar to hotels, you are asked for a credit card at the time you check in and the information will be held securely. This does not compromise your ability to dispute a charge or question what your insurance company has required you to pay. Our billing staff is happy to speak with you about your account. Call 877-989-1234×114. The amount we will charge to the credit card on file will be the portion the insurance company requires you to pay.

When your insurances have paid their portion and notified us of the amount of your share, CCFC will charge your credit card on file and email you a receipt if you like. You can compare the amount charged on your card to the patient responsibility noted on your Explanation of Benefits (EOB) mailed to you from your insurance company. In our agreement with you, when the balance due is more than $ 200.00, you will receive a courtesy call from the billing department and you will have the option of setting up a payment plan. If we are unable to reach you, we will charge up to the agreed amount of $200.00/month until the remaining balance is paid. Common Questions

Why the change?

Things are changing in healthcare, and we need to be sure that patient responsible balances are paid in a timely manner. The credit card on file ensures us that we have a guarantee of payment.

But I always pay my bills, why me?

We have to be fair and apply the policy to all patients. We have wonderful patients and we know that most of you pay your balances. Unfortunately, this is not the case every time.

How will I know how much you are going to charge me?

You will receive a letter in the mail from your Insurance carrier that explains how much of the visit they pay and how much you pay. This is called an Explanation of Benefits (EOB.) This letter tells you exactly, according to your health insurance coverage, how much of your health care bill is your responsibility and how much is your responsibility.

Then what?

We receive the same letter that you do. It arrives about 20 – 30 days after your appointment. We look at each Explanation of Benefits (EOB) carefully, and determine what your insurance has determined as patient responsibility. This is the same way we determine your balance in the past.

But wait, I’m nervous about leaving you my credit card.

We do not store your credit card information in our office. We store it on a secure website called a gateway. We access your information only on this site to process a payment. After we initially store your credit card, we can only access the last 4 digits of your card.

What if I need to dispute my bill?

We will always work with you to understand if there has been a mistake, and we will refund you if we have made a billing error. Please call our billing staff and they would be happy to discuss your bill with you at 877-989-1234×114.

What is a Deductible and How Does It Affect Me?

An annual deductible is the dollar amount you must pay out of pocket before your insurance coverage begins.

For example, if the policy has a $500 deductible, you must pay the first $500 of medical expenses before the insurance company begins to pay for any services.

When does a deductible begin?

Once a year

When do I have to pay for services?

Any time you receive medical care at our office, you will need to pay your full amount.

How will I know when my deductible has been met?

You can call your insurance company at any time to check on how much of your deductible has been met and some insurance companies have this information available online. Every time you receive medical services, you will receive notification from your insurance company with how much they paid or did not pay if the amount went to your deductible.

Will you send me a bill to let me know what I owe?

CCFC does not send patient statements. You will receive an Explanation of Benefits in the mail which indicates the portion you pay and the portion the insurance company covered. Patients are required to keep a credit or debit card on file, but we do not charge anything to your card except the balances noted on your EOB.
